COMPOSITION:-

Each enteric coated tablet contains

Serratiopeptidase JP 10mg

DESCRIPTION:

Serratiopeptidase is an enzyme derived from the bacteria Serratio (the genus serratio) thus named as Serratiopeptidase. This enzyme is derived from these bacteria when the microorganisms are cultivated in the medium containing the milk casein and soybeans milk under specific conditions. After cultivation the highly purified enzyme is prepared through the purification process. As a result the highly purified form is used for medicinal purpose.

CHEMISTRY:

 Serratiopeptidase is an endopeptidase enzyme with a molecular weight of about 60,000.

PHARMACOKINETICS:

    Absorption:

                           I.      Serratiopeptidase is completely absorbed.

                        II.      The maximum level of Serratiopeptidase in the lymph was about 55mcg/ml where as in plasma the concentration is 15mcg/ml (the lymph concentration is more than three times of the blood).

                     III.      The optimum absorption of Serratiopeptidase in between pH 8.5 and 9.5 (the absorption will be maximum in the duodenal pH).

                     IV.      The peak concentration of Serratiopeptidase in both the lymph and the plasma is achieved within one hour.

         

        Distribution:

             I.      Serratiopeptidase is completely distributed in the body.

          II.      The total amount of Serratiopeptidase transported in the lymph is increased proportionately to the dose.

ACTIVITIES OF SERRATIOPEPTIDASE:

It has anti-inflammatory activity (it has the bradykinin decomposing activity, the bradykinins are the cytokines, which initiate the pain at the cells).

Suppresses the inflammatory edema.

Fibrinolytic activity (Dissolves the clot due to the formation of plasmin from the plasminogen).

Caesinolytic activity:

Since it dissolves the milk protein which is present in the milk, this is the drug of choice in breast engorgement (a painful breast with the accumulation of the milk is called as the breast engorgement).

Mucolytic activity (converts the muco-polypeptides into simple mono-peptides and helps in the expectoration of the mucous) acts like a mucolytic and mucokinetic agent.

This is the reason why this enzyme is being added in some cough expectorants in countries like Korea.

Fibrinolytic activity: It combines with the α 2 (alpha 2) macroglobulins and makes plasminogen left free and finally acts like a fibrinolytic agent.

Enhances the antibiotic penetration:

Since it clears all the substances like cell debris, pus, mucous, and blood clots, it enhances the microcirculation and finally enhances the antibiotic penetration.

INDICATIONS OF SPD:

1.      Surgery: In all the surgical interventions there will be surgical trauma leading to inflammation and finally the loss of function. SPD clears cell debris, haemotoma, pus and improves micro-circulation and finally restores faster recovery. In conditions like post-operative inflammation, skin grafts, SPD enhances faster recovery.

2.      Orthopaedics: In all the orthpaedic conditions like fractures & dislocations, soft tissue injuries, sports injuries, and all orthopaedic surgical interventions there will be inflammation and loss of function. SPD enhances faster recovery.

3.      Dentistry: In all the dental indications like tooth extractions, tooth fractures, Maxillofacial surgeries, SPD enhances faster recovery.

4.      Gynaecology: In gynaecological indications like Pelvic inflammatory diseases, Salpingitis (inflammation of the fallopian tubes), oophoritis (inflammation of the ovaries), breast engorgement (painful breast with the accumulation of the milk) and the gynaecological procedures like episiotomy, hysterectomy, caesarian section, SPD enhances faster recovery to the patient.

5.      Paediatrics: In Paediatrics conditions like Epiglottitis, chronic  obstructive pulmonary disease, traumatic injuries, sports injuries, and for the liquefication of the glue in the ear and all other paediatric surgical interventions.

6.      ENT conditions: In all the ENT conditions like upper respiratory infections and also the surgical interventions (Tonsillectomy etc.), SPD enhances faster recovery.

Also in ENT conditions like the formation of the glue in the middle ear, SPD liquefies the glue in the middle ear so that the aspiration of the glue is made easier from the middle ear.

7.      General Medicine: In the field of general medicine, SPD is the drug     of choice in indications like the respiratory tract infections. SPD acts like a mucolytic and mucokinetic agent and it expectorates the mucous very easily and adds in the penetration of the antibiotics for the faster recovery. Also there are reports which clearly states that SPD enhances the penetration of Rifampicin in the treatment of tuberculosis at the tubercular leisons in the lung tissue.

8.        Plastic Surgery: In the field of plastic surgery like skin grafts, SPD helps in faster healing by avoiding the skin graft rejections and helps in proper acceptance of the skin grafts.

DOSAGE:

SPD 10mg 1 tab TID for 7 to 10 days.

 

SPECIALITY OF SPD ( pH Specific tablet)

SPD is available as pH specific tablet (Enteric coated tablet) which dissolves only at a pH of 7.5 to 9 (alkaline duodenal pH) which ensures the maximum proteolytic activity.

SIDE EFFECTS:
The drug has certain side effects, that can affect individuals in different ways. The following are some of the side effects that are often associated with the drug:

• Anorexia
• Gastric Discomfort
• Nausea

Discontinue use of hypersensitive symptoms such as eruptions appear. The drug

should be used with caution by pregnant and lactating women.

CONTRA-INDICATIONS:

 Blood coagulation abnormalities.
